Benefits

– Maximum emergency medical benefit: $2,000,000
– Hospital expenses (semi-private room) limited to 60 days for injury or illness (30 days per policy for psychiatric treatment).
– Medical services inside or outside the hospital and up to 5 follow-up visits. – Diagnostic services: tests that are required as a result of an emergency. Prior approval is required for all major diagnostic tests, including but not limited to MRIs, CT scans, ultrasounds, sonograms, and biopsies. * – Private duty nursing services up to $10,000. – Ambulance services up to $5,000 for a ground or air ambulance to the nearest hospital. – Prescription drugs
up to 30-day supply. – Professional Medical Services – Treatment received from a licensed chiropodist, podiatrist, physical therapist, chiropractor, or osteopath, up to $70 per visit up to a maximum of $700 for a covered injury.
– Medical Devices – Purchase or rental (whichever is less) of a hospital bed, wheelchairs, crutches, canes, splints, slings, trusses, braces, or other prosthetic devices. * – Emergency Evacuation: Up to a maximum of $100,000 per air ambulance, stretcher, or economy class airfare to transport you home as a result of a covered emergency; plus the cost of an assistant, if medically necessary. * – Accidental Dental: Up to $2,500 for emergency dental treatment as a result of an accidental blow to the mouth.
– Emergency Dental – Up to $100 for pain relief and up to $250 per tooth to remove impacted wisdom teeth. (This benefit excludes crowns and root canals.) – Psychiatric Care: Up to $5,000 for the services of a legally qualified psychiatrist while you are an inpatient after an emergency, plus up to five (5) follow-up visits. – Trauma Counseling: Up to 6 sessions for a covered emergency. – Family transportation and subsistence allowance: up to $3,000 for the transportation of a family member and up to $150 per day (maximum $1,500) for meals and lodging in the event of your hospitalization for at least 7 days. *
– Tuition Reimbursement: Up to $5,000 per semester for tuition paid, if you have an emergency that prevents you from attending school and, as a result, you are unable to earn passing grades for the semester, as confirmed by your doctor and registrar of the school you attend. – Repatriation of Mortal Remains: Up to $25,000 to prepare your body for burial or cremation and burial where you die, or the cost of returning your body or ashes home. * – Travel break: During the coverage period, you can return home to attend special events for up to 21 consecutive days without rescinding your policy (you must request and receive prior approval from the Assistance Center). *

Non-Emergency Treatment: – Annual Medical Exam – Up to $100 per year to a doctor for an exam and tests and for a consultation session. – Eye exam: one visit to the optometrist per year. – Accidental death or dismemberment insurance: up to $10,000. * These benefits must be approved and/or arranged in advance by the Assistance Center.
